Oh seem you missunderstood your meaning, if you want to add the image to the page title you can go to admin MGS -> General Settings -> Page Title and upload your image here. But if you would like it to change follow per categories or use the category image as the background of page title, it is impossible, this is my meaning when said you have to pay extra.
